Our team of Clinical Pharmacists work across the practices in the network to support those with long-term conditions like Asthma, COPD and Diabetes, and review patients taking multiple medicines to make sure their medication is working. 

First Contact Practitioners

Our team of First Contact Practitioners work across the practices in the network to help patients with musculoskeletal issues. 

Social Prescribers & Health and Wellbeing Team

Our Social Prescribing Link Workers work across the practices in the network to help patients work towards debt reduction, financial stability, better housing and employment conditions and reduced social isolation. They also provide a well-being service that aims to combat loneliness and isolation in our community.
