SWAGGA PCN offers support for people with mental health needs. Our service is designed to be simple, accessible, and focused on helping you feel better and stay well.
We provide support through our Mental Health Nurses, who can help with low mood, anxiety, depression, and general mental health concerns. They can offer advice, support with managing your symptoms, review your medication, and help with things like sleep and wellbeing. We also provide regular physical health checks for people living with severe mental illness, helping to monitor your overall health and identify any problems early. To access support, you can contact your GP practice. They can help you book an appointment with a Mental Health Nurse if this service is suitable for you. In most cases, you can arrange this directly through your surgery.
Our team includes experienced Mental Health Nurses and healthcare staff who work closely with GP practices to support your care.
