Learning Disability


SWAGGA PCN supports people with learning disabilities to access healthcare. We aim to make GP services more inclusive, comfortable, and easier to understand, so you feel more confident when attending appointments.

We work with GP practices to improve the care they provide, including supporting them to achieve the Learning Disability Friendly Award. This includes offering annual health checks, making reasonable adjustments, and improving communication. We also help practices make changes to their environment, such as reducing noise, improving signage, and providing easy-read information and sensory support items.

We also offer support in the community, including sessions to help people feel more comfortable with medical equipment and GP visits. These can help reduce anxiety and make appointments easier.
