Ageing Well Team – Supporting Health and Independence
At SWAGGA PCN, our Ageing Well team is dedicated to helping older adults live healthier, more independent lives. We understand that ageing brings unique challenges, and our goal is to provide proactive, personalised care that supports physical, mental, and social wellbeing. By focusing on prevention and early intervention, we aim to reduce avoidable hospital admissions and improve quality of life for our patients.
Proactive Care in Practice and Community Settings
Our approach goes beyond the traditional GP practice. The Ageing Well team works closely with patients, families, and carers to deliver care where it matters most—whether that’s in the surgery or within community spaces. We provide comprehensive health reviews, medication checks, and lifestyle support, ensuring that every individual receives tailored care based on their needs and goals.
Why It Matters
Proactive care isn’t just about treating illness—it’s about preventing it. By identifying risks early and offering practical solutions, we help people stay healthier for longer. Our Ageing Well team is committed to making ageing a positive experience, empowering individuals to maintain independence and enjoy life to the fullest.
