Health and Wellbeing Coaches

Health & Wellbeing Coaches provide people with the time to open up and facilitate them to take control of their lives again.
These patients typically are at a lower level of activation so they're not as ready for change. They require time, patience and a safe space.

  • Managing long term health conditions - blood pressure, Diabetes, cardiac
  • Managing mood and mental health
  • Grief and bereavement support
  • Bringing everything together to make it more manageable
  • Medication adherence
