ABCD – Preventing Cardiovascular Disease in Our Community
At SWAGGA PCN, we are committed to reducing the risk of cardiovascular disease (CVD) through early detection and proactive intervention. Our ABCD approach focuses on four key risk factors that significantly impact heart health:
A – Atrial Fibrillation
B – High Blood Pressure
C – High Cholesterol
D – Diabetes
By identifying these conditions early, we can help prevent serious complications such as heart attacks, strokes, and long-term health issues.
Opportunistic Community Health Checks
Our team delivers opportunistic health checks in both practice and community settings, making it easier for people to access vital screening services. Whether at local events, community centres, or GP surgeries, we provide quick and simple checks for blood pressure, pulse rhythm, cholesterol levels, and blood glucose. These checks allow us to spot potential problems before they become serious, offering timely advice and referrals for further care when needed.
Why It Matters
Cardiovascular disease remains one of the leading causes of ill health and premature death. Through our ABCD programme, we aim to empower individuals with knowledge about their health, reduce health inequalities, and create a culture of prevention. By working together with local partners and engaging directly with our communities, we are making heart health a priority for everyone.
