What is SWAGGA Primary Care Network?

SWAGGA Primary Care Network is a collaboration of 16 GP Practices geographically aligned and working together to improve patient care.  SWAGGA PCN has a weighted patient population of approximately 105,000 patients.  It is an extensive Network with a diverse population and patient needs. What is a Primary Care Network (PCN)?

A Primary Care Network consists of groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations. Networks should be small enough to maintain the traditional strengths of general practice but at the same time large enough to provide resilience and support the development of integrated teams.

What are Primary Care Networks designed to do?

Primary Care Networks are designed to provide proactive, coordinated care to their local populations, in different ways to match different people’s needs, with a strong focus on prevention and personalised care. This means supporting patients to make informed decisions about their own health and care and connecting them to a wide range of statutory and voluntary services to ensure they can access the care they need first time. Networks should also have a greater focus on population health and addressing health inequalities in their local area, using data and technology to inform the delivery of population scale care models.

Mental Health & Learning Disabilities Care Coordinator: Annmarie Clarke

Clinical Pharmacists & Pharmacy Technicians 

Our team of Clinical Pharmacists work across the practices in the network to support those with long-term conditions like Asthma, COPD and Diabetes, and review patients taking multiple medicines to make sure their medication is working. 

First Contact Practitioners

Our team of First Contact Practitioners work across the practices in the network to help patients with musculoskeletal issues. 

Social Prescribers & Health and Wellbeing Team

Our Social Prescribing Link Workers work across the practices in the network to help patients work towards debt reduction, financial stability, better housing and employment conditions and reduced social isolation. They also provide a well-being service that aims to combat loneliness and isolation in our community.

Please DO NOT send us personal information, patient information, medical queries, appointment queries, or other queries for your GP Practice, as we are unable to provide assistance in these matters. For these queries, please contact your usual GP Practice or the healthcare service provider.

Our Vision for SWAGGA Primary Care Network

SWAGGA Primary Care Network will act and feel like one organisation whilst respecting the primacy of individual General Practices. We will operate as an equal partner across our health and care system, proactively contributing to improving patient outcomes, and reducing the health inequalities in the population we serve.

We will be known as an excellent place to work. We will attract and nurture the best talent who will form the heart of outstanding services which meet the needs of our patients. We will be proactive in securing diverse income streams and strive to deliver our services from high quality estate. We will utilise digital infrastructure to make our systems more efficient, whilst ensuring that our services remain accessible and inclusive for patients.

  3. Facilitating collaboration amongst all PCN member practices to maximise efficiencies and increase quality of care.
  4. Continuing to improve engagement with local health and care providers, and where necessary, challenging sector colleagues to create a more seamless pathway of health and care.
  5. Empowering our patients and local communities to help us shape local NHS services.
  6. Strengthening our working relationship with Voluntary, Charity and Social Enterprise Sector organisations to optimise usage of our local community resources.
  7. Continuing to provide effective signposting across all services to ensure patients are seen by the right person, at the right time.
  8. Optimising digital resources to streamline patient access to appropriate healthcare, and providing alternatives for those without digital access.
